Steps to request required medical documents for a Patient (Subscriber)
(Medical Provider and Emergency Contact Process)
STEP 1: Medical Provider Actions
- Determine whether the patient (subscriber) has a Protected Personal Data (PPD) Card with a membership number (PPD MemberId), or inquire with anyone accompanying the patient if they possess a Protected Personal Data membership number or know who the Power of Attorney (POA) is to obtain the MemberId.
- Enter your information as a medical provider so the POA can contact you
- Enter the Protected Personal Data membership number of the Patient (Subscriber)
- Check the box to validate you are not a robot
STEP 2: POA - Emergency Contact Actions
- Once request is submitted, the POA Emergency Contact will be contacted via email and text with the contact information above

STEP 3: POA - Emergency Contact Actions
- The POA (Emergency Contact) will reach out to the medical provider listed in the previous steps to understand the situation, validate the medical provider and, if appropriate, share the necessary documents.

Once the medication provider has been validated, the POA should click the "here" link provided in the email.
- The POA will then see the following screen.

The patient (Subscriber) will receive a confirmation email documenting that access has been granted.
STEP 4: Medical Provider Actions

The medical provider will receive an email as shown below. Click the ACCESS DOCUMENTS button in the email.
- Check the box confirming you are not a robot, then click Submit.

Download the necessary documents as needed.
