Steps to request required medical documents for a patient
(Medical Provider and Emergency Contact Process)






STEP 1: Medical Provider Actions




  • Identify if your patient has a Protected Personal Data Card with a membership number (PPD MemberId) or ask the person with the patient, if they have a Protected Personal Data membership number

  • They may need to contact another person who is POA to get this membership number







  • Enter your information as a medical provider so the POA can contact you



  • Enter the Protected Personal Data membership number of the patient



  • Check the box to validate you are not a bot



  • Press Submit







STEP 2: POA - Emergency Contact Actions




  • Once request is submitted, the POA Emergency Contact will be contacted via email and text with the contact information above

STEP 3: POA - Emergency Contact Actions




  • The POA Emergency contact will contact the medical provider whose information was entered in the above steps to understand the issue and makes the call to share the documents if deemed appropriate.

  • As a medical provider please validate your email address at your medical facility with the POA Emergency contact



  • Once this validation is completed the POA Emergency contact click on the here link in the email



  • The POA Emergency contact will see this


  • The Patient will get this email below so a record of giving access is documented in email







STEP 4: Medical Provider Actions




  • The medical provider will receive the email shown here. The medical provider should click the ACCESS DOCUMENTS button shown in the email



  • Check the box that you are not a bot and press Submit



  • Download the documents appropriately
